Systemic: Some of the medicine leaked out, full dose was not given-Mild, Additional Details: Patient was administered a dose of menquadfi, but some of the medicine leaked out from the syringe, not sure of the amount, so she did not receive the full dose, I will discuss with her doctor to see if they want us to readminister the vaccine
