Site: Bruising at Injection Site-Mild, Systemic: Patient did not have adverse reaction form receiving vaccinations-Mild, Additional Details: [withheld] has be made aware that patient specific orders are required for vaccines given to patients younger than 18 years . Discussion has been had that the recommended workflow for these vaccines is for the [withheld] to contact the NP/PA via [withheld] Secure Chat to discuss the scenario, obtain an order, and document that this was obtained in the chart. Unfortunately [withheld] did not contact NP.
