Symptom narrative

At 2:00 pm, the patient arrived with his father to the clinic with an immunization record. Immunizations were transcribed into system by RN. Varicella (Lot# Z016561 EXP: 10/8/2027)and Dtap+ IPV vaccines were offered to the father per recommendation, to which the father agreed. After triage and explaining vaccine side effects, the vaccine was then administered to the child without any incident. Then father and child left the vaccine room. RN was then notified by front desk staff that the father had a question about the Varicella vaccine. RN checked the vaccine record again and noticed that the child had already received two doses of Varicella. The Varicella vaccine on the Immunization card was written with same color of ink with the card margins, making it less visible. RN reassured the father an extra dose of the vaccine will not harm the child, it will provide added protection. The child's parents verbalized understanding and said they were "okay about getting an extra dose". The Immunization MD on call, was notified about the incident via phone call. He stated, "there was no need to be worried and that it will just add protection for the child". The Immunization record was updated with the additional dose of Varicella given on 4-2-26. The client left the clinic with his father in stable condition. PHN Supervisor and PHN Manager were notified of the incident. For mediation it was discussed with RN to double check all vaccine records thoroughly to prevent this from happening in the future.
