Symptom narrative

Client presented on 4/6/26 for repeat vaccines and reported the evening of last administration on 3/5/26 his left arm felt weak and painful (denied redness and swelling) lasting 1 week, also had intermittent periods of dizziness and feeling weal on and off for 1 week. Denied fever. Continued everyday activities did not seek medical evaluation. Symptoms completely resolved. Mutual decision made to use right arm for today's vaccine administration.
