Symptom narrative

Patient received the varicella vaccine on 04/01/2026, no signs or symptoms of an adverse effect. Monitored patient 15 minutes post vaccine. Had instructed patient on adverse reactions before vaccine was given and to seek medical attention if she had any. Patient and Aunt verbalized understanding at that time. Patient's Aunt came in to show a picture of the patient's right arm swollen and red on Friday April 3, 2026. I called the patient directly. She stated her symptoms started on 04/02/2026 at around noon when she noticed redness and swelling in the area of the vaccine injection site from the previous day. She stated she started to feel feverish but didn't take her temperature. She stated she had some itchiness in the area as well. She stated it started to get worse overnight. I advised her to seek medical attention immediately. Monday 04/06/2026: Tried to call patient twice, no answer. Left a message to please call back. No return phone call.
