Incomplete course of vaccination

Symptom narrative

has not received the second dose of the vaccine; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of incomplete course of vaccination in a 88-year-old female patient who did not receive Herpes zoster (Shingrix) for prophylaxis. Previously administered products included Shingrix (received first dose on 15-NOV-2024 without batch number and expiry date). The patient did not receive the 2nd dose of Shingrix. The patient experienced incomplete course of vaccination (Verbatim: has not received the second dose of the vaccine). The outcome of the incomplete course of vaccination was not applicable. Additional Information: GSK Receipt Date: 01-APR-2026 The pharmacist mentioned that it had been over one year since the first dose had been administered. The lot number and expiration date were requested but were not provided. The batch number was not provided, and unable to contact the reporter.
