Symptom narrative

Syncope post vaccine (MenQuadfi) administration. Within 2-3 minutes of MenQuadfi administration mom reports pt slowly falling too his side on the exam table before making a "strange" sound with BUE contraction lasting 30 seconds to 1 min. Eyes remained open and dilated throughout this episode. He was clammy upon becoming alert. Was not responding during the episode. No fecal or urinary incontinence. Hx of seizures as young child. Mom states he has never done this before and this felt different than someone "passing out". He has "passed out" after first dose of HPV vaccine but not to this extent. 60+ min post vaccine administration, pt continues to feel lightheaded with sitting. Unable to obtain sitting vitals d/t pt having to hold himself up to sit and quickly feeling lightheaded upon sitting. He remains pale in color and nauseous. Decision was made to send pt to ED by ambulance as pt unable to sit up longer than 2 mins without lying back down. Vitals are documented and were checked throughout this observation period. Pt was given cold washcloth and sprite. Ambulance departed picked up at 1732.
