The patient's past medical history included shingles (when the patient was 22, the worst pain in 1977) and pain. On an unknown date, the patient received the 2nd dose of Shingles vaccine, the 1st dose of Shingles vaccine and Varicella zoster vaccine. On an unknown date, an unknown time after receiving Shingles vaccine, Shingles vaccine and Varicella zoster vaccine, the patient experienced vaccination failure (Verbatim: suspected vaccination failure) (serious criteria GSK medically significant) and shingles (Verbatim: shingles). The outcome of the vaccination failure and shingles were not reported. A few years ago, the patient received both doses of the newer vaccine. the patient had had shingles twice since then, but it had been nothing compared to 1977. This case was considered as suspected vaccination failure as details regarding time to onset and laboratory test confirming shingles were unknown at the time of reporting.
