Symptom narrative

15 year old pt has not had childhood vaccines. Patient was seen 7/25/2024 and started vaccination catch up process. Vaxelis (Dtap, Heb B, HIb,IPV) was ordered and administered for vaccination. Patient needed all vaccines combined into the Vaxelis vaccine. However, Vaxelis is recommended for patients 6 years and under.
