Symptom narrative

Patient came into clinic on 4/8/2026 2 days after vaccine was given, patient states injection site felt itchy and both upper eyelids have been mildly swollen.

Current illness

Patient had stated she had dental work done and was under anesthesia prior to Medical visit and she did not have anything to eat that whole day.

Glimepiride ( Amaryl ) 2mg

Allergies

Levofloxacin, Penicillin, Nitrofurantoin & Nitrofurantoin and Macrocrystalline.
