Symptom narrative

After immunization, patient developed a localized reaction on his R deltoid above the injection site where he received his MMR injection and bleeding on his L deltoid from Kinrix. Mild erythema and warmth present on L deltoid. No difficulty moving his extremity, chest pain, shortness of breath, s/sx of anaphylaxis. Patient appears stable, NAD. No red flag symptoms present. Patient remained in the clinic with his family for additional 15 minutes and ambulated clinic after without complications or in distress. ER precautions reviewed with parent. Parties were agreeable to plan.
