Underdose

Symptom narrative

Patient was given the incorrect vaccination dosage of 0.5mL, which is formulated for a patient 19 years of age and under, by clinic medical assistant without verifying vaccination information with ordering provider prior to administration. Patient was subsequently requested to return to clinic for an additional IM injection of Engerix 0.5mL to complete recommended adult dosage of 1mL. Patient tolerated both injections well without acute complications.
