On 04/08/2026 I inadvertently administered a booster dose of MCV4 to a child before he turned 16 years old. I called Father and asked how is his son doing; he stated "he is doing well" explained the error I have made and told him he need another dose in 8 weeks from yesterday. so we make an appointment for June 08 at 16;40pm. I also contacted CDC for guidance and they respond " I wouldn't expect a serious adverse event in this situation." My Supervisor has informed the Medical Director and a date will be scheduled for a Root Cause Analysis to be done, to prevent similar errors in the future.
