Symptom narrative

Administration to a 2 MO patient; This non-serious case was reported by a nurse via call center representative and described the occurrence of wrong vaccine administered in a 2-month-old male patient who received DTPa-IPV (Kinrix) (batch number 5c79n) for prophylaxis. Co-suspect products included VAXELIS for prophylaxis. On 19-MAR-2026, the patient received Kinrix. On an unknown date, the patient did not receive VAXELIS. On 19-MAR-2026, an unknown time after receiving Kinrix and an unknown time after receiving VAXELIS, the patient experienced wrong vaccine administered (Verbatim: Administration to a 2 MO patient). The outcome of the wrong vaccine administered was not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK receipt date 20-MAR-2026 The patient received Kinrix, instead of Vaxelis The vaccine administration facility was the same as primary reporter. The reporter consented to follow up.
