Symptom narrative

Infant given DTaP IPV to R thigh 3/20/26. Tenderness mild swelling at site noted on date of vaccination with fever off & on. Swelling at injection site increased over time with pain to touch and crying redness at site. Swelling was always firm & compresses & massaged injection site didn't alleviate swelling nor pain. Pain resolved after several days (7-10) but firm swelling and redness over 1/2 of site persists at 4/8/26 office visit. Firm swelling was 50 cent piece size with 25 cent piece sized erythema with tenderness. At 4/8/26 office visit cellulitis contusion R thigh also noted.
