Symptom narrative

Describe Event, Problem, or Product Use Error: On 1/20/26 the infant was seen in the clinic for a newborn visit. The provider ordered Beyfortus vaccine (Infant RSV) to be administered to the infant. The RN inadvertently administered Abrysvo vaccine (Adult RSV) IM in the left vastus lateralis. The vaccine error was reported within 15 minutes of the event. The family was contacted immediately and instructed to watch for any adverse reaction such as fever, lethargy, poor feeding, or fussiness. The family returned with the infant on 1/21/26 for the provider to re-evaluate the infant. Family denied any adverse symptoms, all vital signs were within normal limits, and child was well appearing. The family consented for the infant to received the correct Beyfortus on this day. Beyfortus vaccine was administered IM in the right vastus lateralis without complications.
