Injection site urticaria

Symptom narrative

Localized urticarial reaction following recent Tdap and meningococcal vaccination. Symptoms mild and limited to injection site. No evidence of systemic allergic reaction or anaphylaxis. In-office interventions-Ice pack applied to right upper arm for 15 minutes with improvement in discomfort. Injection site cleansed with alcohol wipes. Topical hydrocortisone and diphenhydramine cream applied, covered with 4x4 gauze and Tegaderm dressing. Loratadine 10 mL administered in office x 1 dose. Discussed alarm signs and symptoms of anaphylactic reaction. Continue cool compresses as needed. Avoid scratching.
