Symptom narrative

Vaccine administered at 0856 with no adverse signs or symptoms. At 1150, this nurse obtained a subjective history from patient in (withheld) Hospital, emergency department. Patient reported that at 1040 AM, he began to feel hot, with generalized itching and redness over entire area of body with slight difficulty breathing. Pt reported to the emergency department immediately afterwards. Patient's heart rate was tachycardic in 120's but vital signs otherwise stable and within normal range, O2 sats mid 90's.

GERD, Prediabetes, HLD, BKA with RLE prosthesis

Zyrtec 40mg by mouth daily with breakfast
