Mother had voiced to the nutritionist that she was going to have to get the child's immunizations at the local Health Department because the physician did not have the immunizations. The Mother and child were escorted to the clinic room after the appointment. The Registered Nurse went over vaccines that were due at this time. Mother did not agree to all 5 but agreed to 3 vaccines. The RN went and puled the vaccines, ordered and scanned them out and prepared them for administration. Upon entering the clinic room, the mother was on the phone with someone and once the call ended the mother verbalized that she changed her mind and refused all vaccines at this point. Listed vaccines in the VERS report had to be wasted.
