Symptom narrative

Patient reported development of symptoms the evening of the date of vaccination. Patient reported nausea, vomiting, diarrhea, sinus drainage, and swelling of the neck and rash. Swelling has dissipated and rash has started to improve, but patient describes still feeling sick with previously described symptoms for the last week. Patient reported not having this type of reaction to a vaccine before.
