Symptom narrative

Patient stated that after the vaccine was given she felt fine but a week after she noticed her right arm was weak. She stated she couldn't even squeeze her tube of toothpaste with full strength like normal. She then saw her doctor who prescribed physical therapy for her. The physical therapist allegedly stated that they have seen others recently with arm weakness after receiving the TDaP vaccination and said she should inform us.
