Received HEPLISAV-B that was in Transit Too Long and was not Supposed to be Used; Initial report received on 17-Mar-2026. A practice manager reported that a 49-year-old male received HEPLISAV-B (hepatitis B vaccine) for hepatitis B immunization that was in transit too long and was not supposed to be used. No concomitant medications were reported. Medical history included an allergy to penicillin (rash). On an unknown date, the patient received an unknown dose of a hepatitis B vaccine [HEPLISAV-B (lot number 947357, expiration date 07-May-2028, and NDC number 43528-0003-05; route and site not available)]. The patient received HEPLISAV-B that was in transit too long and was not supposed to be used. No other information was available.
