Symptom narrative

Received the 1st Dose 120 Days Ago, and They Never Came Back; Initial report received on 19-Mar-2026. A pharmacist reported that a 26-year-old female received the first dose of HEPLISAV-B (hepatitis B vaccine) for hepatitis B immunization 120 days ago and never came back. No medical history or concomitant medications were reported. On 20-Nov-2025, the patient received dose 1 of a hepatitis B vaccine [HEPLISAV-B (lot number 946062, expiration date 18-May-2027, and NDC number 43528-003-05)] intramuscularly in the left deltoid. On 19-Mar-2026, 120 days after receiving the first dose of HEPLISAV-B, the patient had not returned to receive the second dose. The patient is returning to the health care provider today.
