Symptom narrative

During the patient?s 6-month well-child visit, routine vaccines (Pediarix, Hib, and PCV) were scheduled to be administered. The child?s mother informed the provider that the family would be traveling internationally soon and requested that the patient also receive the MMR and Hepatitis A vaccines. Medical Assistant prepared all ordered vaccines, including MMR and Hib, which require reconstitution. After administering the vaccines, she realized that the diluents for MMR and Hib had been accidentally switched: - The MMR vaccine was reconstituted using the Hib diluent - The Hib vaccine was reconstituted using the MMR diluent The error was identified immediately after administration
