Pediatric doses given to make an adult dose/ after clarification they stated that only one had been administered; Pediatric doses given to make an adult dose/ after clarification they stated that only one had been administered; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of underdose in a 25-year-old male patient who received HBV (Engerix B pediatric) (batch number LG749, expiry date 13-SEP-2026) for prophylaxis. On 01-APR-2026, the patient received Engerix B pediatric. On 01-APR-2026, an unknown time after receiving Engerix B pediatric, the patient experienced underdose (Verbatim: Pediatric doses given to make an adult dose/ after clarification they stated that only one had been administered) and adult use of a child product (Verbatim: Pediatric doses given to make an adult dose/ after clarification they stated that only one had been administered). The outcome of the underdose and adult use of a child product were not applicable. Additional Information: GSK Receipt Date: 01-APR-2026 A pharmacist contacted GSK to inquire about the possibility of using two pediatric doses to administer a full adult dose to an adult patient. During the initial part of the conversation, it seemed as though the health care professional was stating that the two doses had already been administered; however, after clarification, they stated that only one dose had been administered and they wanted to know if they could call back the patient to give the other pediatric dose. The two doses given had the same lot number and an expiration date. The follow-up was declined, as the health care professional stated that they were just covering and did not think the safety team would be able to reach out to them. The vaccine administration facility was the same as the primary reporter.
