given 2 different vaccines at the same visit with Haemophilus influenzae type b in the vaccine; This non-serious case was reported by a other health professional via call center representative and described the occurrence of overdose in a 3-year-old male patient who received Hib (Hiberix) (batch number 547J2, expiry date 27-SEP-2027) for prophylaxis. Co-suspect products included Pentacel (batch number UK373AA, expiry date 31-MAR-2027) for prophylaxis. On 31-MAR-2026, the patient received Hiberix (left thigh) .5 ml and Pentacel (right thigh). On 31-MAR-2026, an unknown time after receiving Hiberix and an unknown time after receiving Pentacel, the patient experienced overdose (Verbatim: given 2 different vaccines at the same visit with Haemophilus influenzae type b in the vaccine). The outcome of the overdose was not applicable. Additional Information: GSK receipt date: 01-APR-2026 The reporter states the patient was given two different vaccines at the same visit with Haemophilus influenzae type b in the vaccine. The batch number was not provided upon follow up with the reporter.
