Patient received his 3 vaccines on his initial visit with us at the-year-old. He did not have a history of any vaccine reactions prior. He does have eczema which the family is dealing with with the 2 other siblings. Mom had seen the allergist which there was testing to be done and reviewed of course. I was in another room with the patient when I was summoned to come back and reevaluate patient mom was holding him rushing him and back into the room he was scratching all over uncomfortable crying getting more flushed this was about a few minutes after the vaccines were given mom was in the lobby. Pulse ox was put on him he was above 95% and he in reevaluating him was not wheezing no respiratory distress but definitively uncomfortable fussing and starting to have some discomfort and breathing mom had some Mobic use in her possession so we went ahead and gave him 1 shot of BQ in his left thigh. Reevaluate him watched him closely over the next few minutes I came back into the room after that saw him continuing to be uncomfortable flailing just crying but he was breathing without any retractions at that point but definitely more flushed and heart rate was in the 180-200 as well as his pulse ox in the mid 90s sometimes dipping to 92% on room air. After another 5 minutes became back and he was continued to deteriorate at that point heart rate consistently at 200s now pulse ox was in the low 90s and 92-93 he his lips were dusky off colored his eyes were being he is becoming more lethargic but still irritable we decided to give us another epi call bring oxygen to the room and run it at 3 to 4 L non rebreatherand decide to call EMS. It took about a few minutes before the ambulance came and at that point he became a little less irritated irritable but continued to have lethargy and his pulse ox continued to be in the mid 90s and his pulse and his heart rate was 180s.
