Vaccine clinic at [withheld name] Elementary School (withheld city, state). Following the administration of the Tdap and Hepatitis A vaccines, the patient experienced vasovagal syncope. Immediately after the Hep-A injection, the patient lost consciousness and slumped over in the chair. Nursing staff responded by assisting the patient into a recumbent position with legs elevated above the level of the heart. Upon regaining consciousness, the patient appeared pale and diaphoretic with cold, clammy skin and reported nausea. Vital signs remained stable: BP 120/80 mmHg, HR 80 bpm, $SpO_2$ 95% on room air, and RR 15. An ice pack was applied to the posterior neck. The patient returned to an alert and oriented (A&O×4) baseline shortly after. The patient was monitored by clinic staff for approximately 30 minutes until symptoms fully resolved. Care was then transitioned to the school nurse on duty for continued observation.
