Symptom narrative

Patient developed a raised, circular rash approximately 4 x 5 inches in size with slight bruising at the left thigh injection site following vaccination. The healthcare provider was notified of symptoms on 4/4, and the patient was evaluated in clinic on 4/4. On 4/7, the patient?s mother reported that the reaction had improved, though a firm area (hard knot) remained at the injection site. On 4/10, follow-up contact with the patient?s mother indicated the patient was doing well, with no ongoing symptoms or additional concerns.
