Inappropriate schedule of product administration

Symptom narrative

Mother called to schedule child's 18 month equipment. The only needed vaccine was Hepatitis A. Administered second dose of pediatric Hepatitis A in right thigh while mother held child. Child tolerated well. Patient and mother left the building. As I was charting the vaccine, I noticed the child was not due for the second dose until 04/30/2026. I reported incident to supervisor via email and I called the mother and explained what had happened and stated that if she wanted to wait until child was receiving kindergarten shots to get another dose she could. Explained that it would still help to give protection to child but it would not be considered valid on the state record. Mother stated she understood.
