Symptom narrative

After receiving order from provider, LPN administered Prevnar 20 and ActHIB vaccines via intramuscular route to 15 month old child at well child visit. Child tolerated administration of vaccines well, mother and child left office without incident. LPN realized ActHIB was expired when documenting and brought to provider's attention who advised LPN to call mother and explain what had transpired, and offer to re-vaccinate in one month. Mother verbalized understanding and chooses to wait for 18 mo well visit for revaccination. Internal incident report has been filed and verbal counseling/re-education plan
