Symptom narrative

Tdap and PCV21 vaccines administered 4/6/26. Patient began to experience headache, fatigue, myalgia, dry cough, and slight redness where he had the shot on upper arm. He presented to the emergency department 4/11/26 for further evaluation. "Left upper lateral arm with some mild erythema no induration nontender. Erythema is approximately 4 x 4 cm oval where he had the vaccination" as per ED attending physical exam. Vaccines were documented to have been administered in right deltoid per vaccine administration notes. PCR respiratory panel was ordered by ED attending and positive for influenza B. Patient was discharged with oseltamivir. Diagnosis "Reaction to pneumococcal vaccine, Influenza B" per ED attending note. VAERS submitted out of abundance of caution.
