Symptom narrative

Service member (SM) received routine anthrax vaccine on 4/10/26 around 1300. SM noticed injection site reaction later that evening. At vaccine site on right arm skin is red, warm, tight, itchy, swollen. By 4/12, symptoms worsened and rash spread down to just above the elbow. SM was evaluated by writer (RN) and clinic MD. It was determined that SM had a vaccine reaction and will be medically exempt from receiving any additional anthrax vaccines. SM was instructed to take NSAID/APAP and Benadryl PRN to manage symptoms. Severe reaction symptoms were discussed with SM, and he was instructed to call 911 if symptoms worsen. Incident was documented in SM medical record.
