Symptom narrative

Patient came into the pharmacy for a consult with the pharmacist on Sunday 4/12/26. Pulled up left sleeve of shirt showing a very red, swollen upper arm. Patient states he felt discomfort while receiving one of the vaccines (patient was unsure of the order in which received) but was ok that evening. Was extremely fatigued on Saturday 4/11/26, resulting in sleeping most the day. Said he felt a little pain in his arm but continued to "rub it". Wasn't until Saturday evening when he lifted up his sleeve, noticing the redness and swelling. Was asked if they had tried any remedies, such as OTC pain relief, patient responded with that they had not. Was also asked if they were experiencing any other types of side effects/reactions ( i.e. shortness of breath, fever, difficulty breathing/seeing) patient replied with no other side effects/reactions. Patient was informed to try OTC Tylenol and if that didn't provide any relief, to reach out to provider within the next day or two. Patient stated he already had a prior made appointment with his PCP for Wednesday, 4/15/26 and would discuss this with them if it was still bothersome.
