On an unknown date, the patient was vaccinated with one childhood dose of Varicella virus vaccine live (oka-merck) (VARIVAX) powder and solvent for solution for injection/infusion, 1 dosage form, total, as prophylaxis (dose, route of administration, anatomical location, lot number, and expiry date were not provided).
