Symptom narrative

Site: Pain at Injection Site-Mild, Systemic: Dizziness / Lightheadness-Mild, Systemic: Flushed / Sweating-Medium, Systemic: Nausea-Medium, Additional Details: Immediately after recieving vaccine, patient reported burning sensation at injection site and quickly escalated to dizziness, lightheaded, nausea. Patient leaned over and wanted to lay down. She seemed fatigued and I had to ask her questions twice for response. She kept closing her eyes and I grabbed the emergency kit and called EMS due to rapid onset of symptoms/new symptoms. Ambulance and police arrived to site and patient stated she had recovered and refused treatment.
