Symptom narrative

Patient reports via phone; headache, chills, n/v, dizziness, left sided chest pain, dull ache 4.5/10 pain for approx 2 days post Covid vaccine. Currently still experiencing intermittent and brief episodes of "less than a minute" CP with no noted aggravating/alleviating factors. Previous covid vaccines reportedly caused nausea, body aches and chest pain which subsided with no lingering complications after her initial doses. Patient reports she did not go to urgent care or ED, is unable to be seen in clinic for 8 more days due to moving. Scheduled with provider to be evaluated on 4/21. Instructed for worsening symptoms to schedule for earlier date or seek emergent care.
