Symptom narrative

Patient developed a red, circular rash at the injection site on his right thigh approximately 16 hours after receiveing Dtap/IPV immunization. The area was warm to touch and tender. Area grew in diameter so patient was taken to the emergency room and given a course of tapered prednisone and advised to take an over the counter antihistamine. Patient was also advised to take acetiminophen and/or ibuprofen as needed for discomfort and pain.
