Symptom narrative

Patient was given the vaccine in the clinic and then was observed in clinic 15 mins after administration. No signs of adverse reactions were observed or noted. Once it was noted that an error had occurred, the mother was notified immediately and another appointment was made to return for the correct vaccine. Mother stated to front office personnel that patient had shown no signs of reaction to the vaccine, and was fine . The vaccine that should have been given was the MCV4 vaccine for college.
