Reported Symptoms: 10037844:RASH; Narrative: Other Relevant HX: 1/5/26: SCHEDULING NOTE;: Patient called to request call back from RN to discuss vaccination site from last week. Patient reported that site is sore with slight redness. Patient did not wish to speak to symptom line at this time and stated that she was "not overly concerned, but would like to report and discuss with nurse". 1/5/26: Telecare note: (patient) States it is sore with slight redness. She states that in the arm where she had her pneumonia and RSV vaccines she has had redness that has now spread to the full circumference of her arm. She states that she's had strange reactions to vaccinations in the past, this one looks like it originates where she had the pneumonia vaccination. She takes cetirizine daily, but the redness has still spread. RN advised that provider is out today , she will go to urgent care for evaluation 1/5/26 ER visit note: Chief Complaint: Patient states on 12/31 she had RSV and pneumonia shot in L upper arm. Patient states she left and noted aching at vaccination site. Patient states, she expected the aching, but on 1/1/26 she noticed mild swelling and redness. Patient states the swelling and redness have increased. Patient states pain has gotten a little worse as well. Patient denies fever/chills, SOB, facial swelling, CP, burning/numbness in arm below injection site or history of vaccine reaction. 1/5/26 ER RN note: Triage RN Note (Subjective/Objective): Patient alert and oriented; GCS 15. 12 cm band of redness/swelling noted on L upper arm. L Radial pulse palatable; L hand warm
