Symptom narrative

Patient came to get 2nd dose of Shingrix. Upon checking State Immunization Database and there was no records for Shingrix or Prevnar 20 so in addition, Prevnar was offered to her. After the shots were given, patient requested to have a completed records list from State Immunization Database printed out and when I tried to print out, the shots did not link up her State Immunization Database profile for some reason. Multiple attempts at doing unsold to re-release were attempted but shots could not be linked up so they were manually added in. As the shots were added in, I found records of 1st dose of Shingrix (12/04/2025) and Prevnar 20 (09/30/2025) ONLY in system by going back records for up to 1 year as it only defaults to 3 months. Those shots were manually added in as well upon discovering.
