Tdap given at 9:13AM. Patient felt fine until about 9:27 when she needed to go to the bathroom. Stated she had diarrhea and nausea. Provider in to see patient, phenergan 25 mg given IM at 9:36. At 9:44 (Vitals:99 / 105/ 132/93) patient stated that she began to feel increasing shortness of breath, stridor ausculated on neck. Epipen adminstered. She immediately felt better after epipen, nausea mostly resolved. EMS called. Vitals 128/68 9:56 HR 116 sat 99, 10:04 EMS arrived. Report given. Patient transported to ER.
