Patient was vaccinated at outside facility. Patient did not remember the name of the clinic he went to. State vaccination record data reports organization as "[withheld]". Data shows one dose of Hep B vaccine given. Patient told me that he had received "Hep B in one arm and Hep A in the other", no record of HepA vaccine given on state record. Hep B vaccine was given on 2/12/26. On 2/23 we spoke to the patient, and he did not report any abnormalities. On 3/10 he called in to the pharmacy complaining of "joint pain" and requested for us to screen his medication for side effects. RPh did so and advised the patient to take an OTC pain reliever. On 3/19 we spoke to him and he reported still having joint pain but declined RPh consultation. On 4/2 he requested RPh consultation to address difficulty walking that he had been experiencing (first report at this time); RPh provided consultation and advised patient to seek further care. On 4/14 we reached out to the patient and he informed us that he had been diagnosed with Guillen-Barre syndrome and was currently hospitalized. He attributes the condition to his vaccination in February which we were unaware of. He reported that he was admitted on 4/2 (same day as last contact) spent 4 - 5 days in hospital until he was transferred to a rehab center where he is currently. He reports that he is paralyzed from the waist down but hopes to recover.
