Symptom narrative

pt administered vaccines on 4/10/2026, after returning to school, school nurse called stating vaccines administered on 9/9/2025 were invalid. That did not show on our end and was not picked up by EPIC. After reviewing the chart noted that the MMRV would have to be repeated. Notified the father and reviewed information with the father and pt came on 4/14/2026 to re-administer vaccine. Vaccine given on 9/9/2025 lot number is Z010379.
