Patient rec'd vaccine on 4/7/26. Husband called 4/9/26 to report adverse event including diarrhea, dizzy, fatigue, and speech change. Was triaged by nurse and scheduled for appt on 4/10/26. Patient was deceased prior to appt on 4/10/26.
