Symptom narrative

4/29/25 - Patient reports left shoulder pain since receiving Hep A and Hep B vaccines; reporting sleep disturbances; constant aching pain with occasional sharp pains; pain radiates through arm and into neck and upper back. No relief from OTC meds, rest, or application of heat or ice. 4/3/26 - Patient reports ongoing left shoulder pain with aching, constant pressure, a pulling sensation, occasional stabbing pain, and periodic weakness, and limited range of motion. She has tried Celebrex, Baclofen, Naproxen, and Ibuprofen with some benefit; two cortisone injections provided minor relief. No other trauma or injury has been reported. Additionally, patient reports numbness in the first three digits of the left hand for the past 3 months.

Current illness

Seen by PCP on 3/17/25 with symptoms of cough, congestion, sore throat, and fever - Negative for strep, Covid, and Flu.

Medical history

Hepatitis C

Other medications

Apple Cider Vinegar; Hydrochlorothiazide; Ibuprofen; Inulin; Multivitamin with Folic Acid; Zepbound; Valacyclovir HCl.

Allergies

No known allergies.

Lab data

Imaging from x-ray and MRI show mild AC osteoarthritis and no acute osseous abnormalities.
