Erythematous patch noted on right thigh injection site measuring 8.5 by 15 centimeter, blanchable. Area indurated and warm to touch, more tense centrally. Oval-shaped lesion. Induration more prominent medially and centrally. No fluctuance palpated. Plan: Plan: - Hydrocortisone 1% cream applied twice daily to affected area - Cetirizine 2.5 mL daily as needed for itching - Ibuprofen 14 mL every 6 hours as needed for inflammation - Continue warm compresses as they provide relief - Gentle massage of the area may be helpful - Avoid direct ice application to skin; if cooling needed, use cool rag with cold water as barrier - Return if reaction worsens or does not improve by end of week for consideration of short-course oral corticosteroid
