No adverse event. Patient was administered DTap when provider originally ordered Tdap. Incorrect vaccine was given. Provider spoke with patient today and patient reports doing well. Per provider does not need to return to clinic.

Current illness

Diarrhea on 04/13/26. Epigastric abdominal pain on 03/17/26.

Medical history

COPD, HTN, Alcoholic Cirrhosis of liver without ascites, and history of colon, ovarian, and uterus cancer.

Other medications

Montelukast 10mg tab, Polyethylene glycol 17 gram/dose powder, and Senna 8.6mg tab.
