Symptom narrative

Pt went to urgent care for evaluation of arm. Experiencing red rash in area of injection. Reports skin area was hard, warm and bumpy. Also reports foot was sore for a few days. Urgent care diagnosed case as skin infection caused by injection and prescribed antibiotics.
