No additional AE; Nurse called to report that GARDASIL 9 was inadvertently administered to a patient after being improperly stored.; It was shipped on April 7, 2026 and did not arrive until April 13, 2026, and the distributor told the office it was not stable to use.; This spontaneous report was received from a nurse and refers to 32-year-old female patient. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 13-Apr-2026, the patient was inadvertently vaccinated with Human Papillomavirus 9-valent Vaccine, Recombinant (Gardasil 9) (valid lot #A003663, expiration date: 11-Dec-2026) 0.5 mL as prophylaxis after being improperly stored (product storage error). It was shipped on 07-APR-2026 and did not arrive until 13-APR-2026, and the distributor told the office it was not stable to use (product distribution issue). It was however administered before they received the notification. No additional adverse event (AE) was reported.
